Devoted Creations Rebate Policies and Guidelines
Your Salon must be entitled to a minimum $20 rebate for your request to be processed, smaller rebate requests will not be processed.
Salon Rebate Form must accompany your purchase invoices and an executed W-9 form.
The invoice must include the following information:- Date
- Customer name or number
- Product names
- Quantity purchases
- Unit prices
- Invoice Number
- Distributor name
Final deadline to submit invoices and redeem product credit for rebate is August 31, 2008.
Rebate requests must be received with a completed 2007-2008 Rebate Form between July 5, 2008 and
August 31, 2008 or it will not be processed. No exceptions!!!
Salon and Manufacturer participation are voluntary.
